THIS QUALIFYING QUESTIONNAIRE IS FOR SINGLE PARENTS SEEKING A SCHOLARSHIP FOR A CREDENTIAL OR CERTIFICATION. THIS WILL NOT BE ACCEPTED FOR THOSE SEEKING AN ASSOCIATE OR BACHELOR DEGREE.

Single parent selected for financial assistance will meet the following criteria: – Is a single parent (see definition below) – Resident of Benton, Carroll, Madison or Washington County – A legal United States resident or a resident with DACA status – Has a high school diploma or GED – Has low to moderate income, typically not more than 250% of federal poverty guidelines – Enrolled in or plans to enroll in a program of study at an approved institution – Has completed or is in the process of completing the FAFSA (as applicable) – Maintains a cumulative grade point average (GPA) of at least 2.0

*For purposes of the SPSFNWA Program, “single parent” is defined as: a family with children under the age of 18 and/or a severely disabled adult child over 18, headed by a parent who is widowed or divorced and not remarried, or by a parent who has never married and is caring for the children without assistance of another parenting partner in the home.
