Additional Requirements

Please refer to the list of additional requirements associated with the TYPE of scholarship (Traditional, Allied Health, or Professional Certification) you are applying for and your STATUS (first-time applicant, previous applicant with incomplete application, or current recipient).

FIRST TIME APPLICANTS

(This includes previous applicants with incomplete applications and returning applicants who received scholarships prior to 2016)

Submit the following supporting documents to our office at 16 W. Colt Square Dr., Fayetteville, AR 72703, in addition to the online application form:

  1. Three letters of reference from people (not related to you) who are familiar with your life experiences and with your character.
    *Must include COMPLETE contact information (name, address, telephone, email)
  2. A personal goal statement explaining why you chose this particular course of study and what you hope to achieve. Feel free to include any information about yourself which might be helpful to the Selection Committee in its evaluation.
  3. A copy of your high school transcript OR your GED certificate and test scores.
  4. Official transcripts from any colleges or schools you have previously attended.
    *Please note: an OFFICIAL transcript must be sealed (unopened) and stamped or signed by the registrar.   Faxed transcripts are not considered official.  EScript-safe transcripts will be accepted.
  5. A letter of acceptance/admission from the school of your choice (or an official transcript that indicates current enrollment).
  6. A copy of your valid(unexpired) photo ID. International students must submit a green card which indicates their status as a legal resident, and expiration dates.
  7. A copy of your most recent federal tax return (form 1040).

CURRENT RECIPIENTS

Submit the following supporting documents in addition to the online application form.

  1. An official transcript *Fall scholarships Only
    *Please note: an OFFICIAL transcript must be sealed (unopened) and stamped or signed by the registrar.   Faxed transcripts are not considered official.  EScript-safe transcripts will be accepted.
  2. A copy of your most recent federal tax return (form 1040) *Fall scholarships only.
  3. A copy of your valid (unexpired) photo ID.  International students must submit a green card which indicates their status as a legal resident, and expiration dates. *Fall scholarships only

Allied Health SCHOLARSHIP APPLICANTS

Submit the following supporting documents to our office at 16 W. Colt Square Dr., Fayetteville, AR 72703, in addition to the paper application form.

PROFESSIONAL CERTIFICATION SCHOLARSHIP APPLICANTS

  1. A personal goal statement explaining why you chose this particular course of study and what you hope to achieve. Feel free to include any information about yourself which might be helpful to the Selection Committee in its evaluation.
  2. A letter of acceptance/admission from the school of your choice (or an official transcript that indicates current enrollment).
  3. A copy of your valid (unexpired) photo ID. International students must submit a green card which indicates their status as a legal resident, and expiration dates.
  4. A copy of your most recent federal tax return (form 1040).

WHAT HAPPENS NEXT?

Upon submission of your application you will receive an email if your application is incomplete.  You will only receive one notice if you are missing required items.

After the submission deadline, applications will be screened for eligibility. Those applicants eligible for a Single Parent Scholarship will be invited to a personal interview. You will receive an email notice telling you to sign up for your interview time. If you miss your interview or do not schedule your interview for any reason you will automatically become ineligible for the scholarship.

After the interviews, applicants will receive a written and email notice advising them whether they have been awarded a scholarship. If you receive a scholarship your notification letter will specify the date scholarship money will be distributed and the procedure to follow to receive your check. Prior to check distribution each recipient must submit a class schedule for the semester covered by the scholarship and a copy of the final grades from the previous semester (if recipient was in school).

Part time students must take a minimum of 6 credits hours to be eligible for a scholarship.

Applicants/students must maintain a cumulative GPA of 2.5.
